Medical at Nationwide Health system in Baguio for Canada
Hello everyone! I hope my experienced during my medical at Nationwide Health System in Baguio for Canada will help you specially for first timers like me!! :D

Second: Idownload lahat ng forms sa website nila, kc hinde ka nila papayagang pumila pag hinde complete ang requirements. Fill up mo na rin para deretchu na.
Third: I went at their clinic at exactly 5:00 am. Pag dating ko dun pang lima na ako sa pila, Around 7 nagbukas na ang clinic nila, mahaba na rin ang pila. Kahit una ka c pila, priority pa rin ang senior citizens at nag pa appointment online. Kaya i suggest magpa appointment na lang kayo online para sure na first kayo sa accomodation, sa appointment lang kc dapat padala mo na rin yung bayad sa kanila. Kaya ang ending naging pang no. 9 and 10 na kmi ng baby ko. Hinde kasama sa priority lane and baby..
Fourth:Hnide ka nila papayagang pumila pag hinde complete ang requirement mo, pakibasa na lang sa baba ang requirements. Pag nakapasok ka na tatakan nila yung kamay mo na peede ka ng pumasok, pwede rin magdala ng isang kasama para my mag aasist sayo lalo na pag my kasama kang bata or matanda.
5th: Around 8:00 sinabi na i-cut na yung pila hanggang 50, kaya marami ng babalik na lang ulit mextime.. Kaya dapat talagang maaga ka.
6th: hintayin tawagin ang pangalan, tatanungin kumg my piercing, tattoo, menstruation or nagpasalin ka ng dugo. Pag Oo mag aaditional test ka oara Hepa B and C meaning additional 700.00 din.. sisingilin ka na rin nila dun sa first step. Nasa baba a rin list ng pyment paki tignan na lang. I paid 10,980 bayad namin ng baby ko, 2200 peson ang medical niya.
2nd step: Hintayin ulit tawagin ang pangalan, for picture and signature pra sa record nila. Mejo natagalan ako kc ang hirap pirturan ni baby, kc dapat daw close mouth, binagyan ko na lang xya ng food para magclose mouth.hahaha
3rd: kukunan ka ng dugo para sa HIV test, Hepa A and C , after kukunan ka na rin ng ihi at chest xray.. magkakatabi ung room nun kaya mabilis lang.. Hintayin ibigay sayo ang papers mo tapos sasabihin ka ng pumunta sa 2nd floor para sa physical examination.
4th: Kukunin ang Height and weight mo, tapos itetest ang mata using di snellen chart.
5th: Physical examination. Magtatanong lang c doc ng history mo ng sakit kung meron. Dahil lalaki yung doctor naiwan yung panty at bra ko sa physical examination, my assistant nmang babae sa loob. Umikot lang ako.tapos na.. tapos c baby, tinanong lng ano kaya niyang gawin at sabihin tapos na..
6th: pupunta na sa station kung saaan sasabihin ang result kaagad..Mejo mabagal kc irereview pa nung doctor tapos tatawagin isa isa.. eexplain nya yung result kung acceptable lahat ng result, kung acceptable ipapadala na nila yung result 1-10 days sa embassy, pag may problema naman you will go another test and go medication tapos babalik sa sasabihin nilang schedule.. So far Acceptable naman ng result.. lastly, Hahanapan ka nila ng MMR vaccine or measles vaccine, kailangan daw yun kaya pakidala na lang ng record mo. Thanks

REMINDERS:
1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state.
2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
3. Applicants who have been scheduled for additional examinations must come on their designated examination date and time.
4. Please bring your prescription spectacles/eyeglasses or contact lenses if applicable.
REQUIRED DOCUMENTS:
A. COMPLETELY FILLED UP DOWNLOADED FORMS (Please download and fill up forms to facilitate processing)
B. 3 pcs picture passport size (recent photo within 6 months old, white background)
C. 3 photocopies of letter from the embassy (IMM 1017), unless Upfront Medical
D. Original Passport

E. 3 good quality photocopies (preferably in colour) of Passport/Valid ID’s clearly showing photo, name and date of birth

- Old chest x-ray films not less than 3 months prior to the medical examination
- Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following inf
- duration and date of treatment
- surgical procedure or medical intervention done
- complete final diagnosis
- outcome of treatment or treatment plans and prognosis, if available
- Vaccination Record (must contain the following details):
- personal copy (e.g. baby book)or copy of medical chart indicating received vaccines
- complete date of receipt of vaccination
- must include signature (license number, if applicable) of doctor or health worker who administered vaccine
